Training and Clinical Practice of Clinical Psychologists in Pakistan: A Descriptive Study of Professional Preparation and Practice Realities

Abstract

Clinical psychologists can be very influential in dealing with the increasing mental health burden in Pakistan; nevertheless, their training and practice is in a disjointed and loosely controlled system. The current study is a qualitative descriptive study which investigated training experiences, supervision, and practice realities among Pakistani clinical psychologists. The data were collected through semi-structured interviews, and the sample consisted of 17 practitioners with the experience of 2-7 years. Reflexive thematic analysis was applied to analyze data. Results showed a strong theory-practice gap in scholarly training, the restricted exposure to supervised clinical practice, and vast differences in urban and peripheral institutions. The participants cited disjointed and subordinated supervision, sudden shifts to autonomy in practice, and learning on the job. Lack of a centralized licensing and regulatory body added to the lack of regulation, loss of professional identity, and lack of public confidence. The employment in a market-based manner, preponderance of the private practice, and constant negotiation of the cultural stigma and family participation further molded clinical work. The paper also highlights the urgency of standard training pathways, mandatory supervision, formal regulation, and increased integration of clinical psychologists into the general healthcare systems to support the workforce on mental health in Pakistan.
